Each test taker is required to read and sign the Confidentiality and Regulations Agreement (Appendix D).
Completing this form will indicate agreement to the conditions referenced in the statement. The test administrator
must retain the completed Confidentiality and Regulations Agreement in a secure location until the start of the third
calendar month after the test date. (For example, retain the forms from October test dates until the beginning of
January.) Then, destroy them by shredding. Do not send completed Confidentiality and Regulations to ETS. If ETS
needs a particular form for an investigation, we will contact the test center during the retention period.
Provide a pencil and one piece of 8 ½-inch by 11-inch scratch paper to each test taker. If test takers have filled both
sides of the scratch paper before the test is completed, you may provide an additional piece of scratch paper. When
the test taker completes the test, collect and destroy the scratch paper.
Your ears must remain visible throughout the test and not be covered by hair, a hat, or other items.
Religious headwear is permitted if your ears remain visible.
Face masks may not be worn during check-in or any part of the testing session.
Dress appropriately for your test. You will not be permitted to take the exam if you are dressed inappropriately.
Avoid wearing items such as jewelry, tie clips, cuff links, ornate clips, combs, barrettes, headbands, and other hair accessories.
There are no scheduled rest breaks during a standard test session. If a test taker must leave the testing
room briefly during the test session to go to the rest room or take medication, the 2 ½ hour limit will continue to
count down. Remind the test taker that they will not be allowed extra testing time for such an absence. When the
test taker returns to the testing room, the session will continue to be available up to the 2 ½ hour limit.
Test takers who are approved for accommodations, such as extra breaks, will have received a confirmation letter
from ETS that states the test taker’s information and the accommodation they are approved for. Please see Chapter
6 (page 30) for information on how to provide extra breaks for test takers who are approved for the
accommodation.

pencils. The use of any of the following items in the testing room is strictly prohibited:

of holding or safeguarding test takers’ personal possessions. Anyone found using testing aids during the
administration (other than electronic devices) should be warned and, if the action continues, should be dismissed
from the examination. Test takers with electronic devices should not be given a warning but should be dismissed
immediately. See “Dismissal for misconduct” for further information (page 25).
